Instructions:

  1. Complete the application form. In order to be considered, your application form must include all requested attachments. Application must be typed.

  2. Provide evidence of your tax exempt status.  It is usually done through a copy of the most recent determination letter from the IRS recognizing your tax exempt status under section 501(c)(3) of the Internal Revenue Code.

  3. Submit two (2) copies of the application, along with the attachments outlined on page 3 of the application.
