Instructions:

Please ensure you complete all steps of the application process. This includes the online application form and submitting the additional required documents.

1. Complete the online application form by clicking the link below. Make sure you provide all requested information to be considered. Select Jasper Community Foundation from within the form when asked for the county/organization.​

2. Submit a copy of the following forms that pertain to your application​. These additional documents may be completed and emailed to JCFoundation@outlook.com OR printed and mailed to the address below.

  • Most recent determination letter from the IRS recognizing your tax exempt status​

  • Most recent tax filing (Form 990)

  • Signed applicant board of approval agreement - included in Additional Documents link below

  • Signed Fiscal Sponsorship Agreement (if applicable) - included in Additional Documents link below
